I Am A Client - How Do I Complete A Treehouse Form?

1. If you are a client of a provider who is using Treehouse, you may receive digital forms to complete from your provider.

2. If a provider has sent you a form (i.e. consent, intake), you will receive an email notification with the following information:

• Subject: Form Completion Required
• Sender: treehouse@em7644.thapi.ca
• Content:

3. Click on “link” in the email to open the form. The form will open in a new tab and you can enter your information. All Treehouse forms are hosted on HIPAA-compliant and secured servers. Your personal information is never shared and is secured.

4. Once you have entered all your information, click Send to directly submit the form to your provider.

5. Once submitted, you will receive a confirmation.
